Before submitting a credentialing application, you or your group must have a signed contract with Martin’s Point Health Care. If a contract is not yet in place, please e-mail to inquire about a contract before submitting a credentialing application.

Provider Credentialing Instructions

Complete the Council for Affordable Quality Healthcare (CAQH) application. Make sure all information and attached documents have been updated and (re)attested. Authorize Martin's Point Health Care to view your CAQH information.

Once the CAQH information is up-to-date, please use our Provider DataPoint, our web-based data tool, to add the provider to a practice/group. Provider DataPoint will determine if credentialing is needed and ask for the appropriate information. If credentialing is required, please allow up to 90 days for processing.
Please note: Effective 06/01/16: Updates and data changes are no longer accepted by email.

Martin's Point credentials the following provider types:

  • MD, DO, DDS, DPM, OD, DC, AuDand Certified Nurse Midwives (CNM)
  • Nurse Practitioners (FNP, ANP, PNP) and Physician Assistants (PA) who practice as a PCP

Martin's Point does not credential:

  • PT, OT, ST, SLP practitioners (Martin’s Point credentials the group)
  • Providers who practice exclusively within the inpatient setting (e.g., hospitalist, laboratory personnel)
  • Nurse Practitioners and Physician Assistants who practice as specialists
  • Covering Physicians (e.g., locum tenens, per diems)
  • Martin’s Point does not contract with Chiropractors (DC) for the US Family Health Plan.

Martin’s Point does not credential behavioral health providers. All inquiries regarding behavioral health providers should be directed to the Behavioral HealthCare Program at 1-800-538-9698.
